What is a Physical Therapist?

Physical Therapists provide services that develop, maintain and restore people’s maximum movement and functional ability. They can help people at any stage of life, when movement and function are threatened by ageing, injury, diseases, disorders, conditions or environmental factors. Physical therapists help people maximize their quality of life, looking at physical, psychological, emotional and social wellbeing. Physical Therapists work in the health spheres of promotion, prevention, treatment/intervention, habilitation and rehabilitation.

What do I need to bring?

Be prepared to provide your previous medical or surgical history. If you have them and if they are relevant bring any and all images such as MRI/X-Ray/CT Scans, Insurance card and Drivers License. You may wish to dress comfortably.

How many visits will I need?

The number of treatments will vary depending on the diagnosis and the individual. After a thorough assessment, the Physical Therapist will discuss the individualized treatment plan for the rehabilitation process.

What should I wear to my appointment?

Dress comfortably allowing access to the body part that will be treated.

How do I make an appointment?

Preferably contact us by phone (386.671.2626) or use the form under the contact page.

Do I have to be referred by a doctor?

Yes, a referral from any physician (MD or DO) or nurse practitioner or physician assistant is required due to insurance and state policy unless a patient is cash pay.

Treatment duration depends on your diagnosis, the severity of your impairments, your past medical history, and other factors.  Re-evaluations will be done to assess your progress and your therapist communicates with your physician to help you achieve your therapy goals.

Physical Therapists are educated and highly trained to treat a wide range of conditions and diagnoses.  Please visit our Services for an extensive list of conditions and diagnoses we see patients for.

Our Insurance Specialist will check with your health insurance provider to determine if outpatient physical therapy services are covered under your plan. We recommend that you confirm with your insurance plan.

It is recommended to call us 24 hours prior to appointment for any cancellation or rescheduling. You may reach us at (386) 671-2626.
